After a discussion of your circumstances and areas of support, an initial assessment is carried out in which we will suggest the best form of support for you/your loved one and you will have full control on what kind of care and support you'd like to receive.
A person-centred care plan is carried out and the support will begin whenever is suitable for you. During the care plan, we will discuss with you what outcomes you wish to receive from our support such as a more social lifestyle or being able to enjoy the simpler things more easily.
